How do I create a form?

Start a new form, give it a title, and choose public or private visibility.

Creating a form takes a few seconds — you give it a title and choose who can respond, then you land in the builder to add your fields.

Create a new form

  1. Open Forms from the sidebar.
  2. Click Create form. The Create form dialog opens.
  3. Enter a Title, and optionally a Description.
  4. Choose a Visibility:
    • Public — anyone with the link can respond.
    • Private — only people with a private link can respond.
  5. Confirm. You will see a Form created confirmation and can start building the form.

Public forms are a paid feature

Public visibility relies on the paid public forms entitlement. If your plan does not include it, you will see an upgrade tag on the Forms list and the public link will not go live. Choose Private to use single-use private links instead.

Public vs private

  • Public forms expose a single shareable link that anyone can open and submit. This is best for open sign-ups, surveys, and requests where you do not know the audience in advance.
  • Private forms are reached only through single-use private links that you generate one at a time. This is best when you want to control exactly who can respond.

You can change how a form is shared later from the Share tab — see Share a form.

What happens next

A new form starts as a Draft and does not accept responses until you Publish it. From the list you can also View form, Build form, Copy public link, Duplicate, or Delete a form using its row actions.

Forms can be duplicated — a quick way to reuse an existing form as the starting point for a new one. Use Duplicate from the form's row actions.
